Kentucky State Police say a van driven by Charles Carwile of Leitchfield, was parked on the right shoulder of I-65, when Carwile attempted to cross three lanes of traffic to make a U-turn Northbound.
A car, driven by Leshanna Shirley of Glasgow, was traveling southbound in the fast lane when the van pulled directly in the lane she was traveling in.
Shirley's car was unable to avoid collision with the van, and struck the driver's side door.
Carwile's van then became caught on fire after the Carwile and his passenger, Bessie Boone of Caneyville, were removed from the vehicle by other motorists.
Shirley's car came to a final stop on an embankment on the left shoulder of the road.
The driver of the van, Charles Carwile, was pronounced dead at the scene by the coroner and his passenger, Bessie Boone, was life-flighted to Vanderbilt Hospital, where she was pronounced dead prior to arrival.
The driver of the car, Leshanna Shirley, was taken to the Bowling Green Medical Center where she is being treated for non-life threatening injuries.
