A maintenance supervisor at Six Flags Kentucky Kingdom says workers did not follow instructions on the ride that severed the feet of a teen last summer.

Maintenance Supervisor John Schmidt said in a deposition in November that technicians for the theme park never performed a hands-on inspection on the Superman Tower of Power ride.

He also said they didn't follow several of the ride manufacturer's handling instructions.

Cables on the ride snapped on June 21, shearing off the legs of 14-year-old Kaitlyn Lassiter of Louisville.

Lassiter's right foot was re-attatched, but her left leg had to be amputated.

Her family is suing the amusement park, which has denied liability in the accident.
