A state investigation is complete and a report is issued, six months after the Kentucky Darby Mine explosion that killed five men in Harlan County.
The main thing found in this report is proof the mine was unsafe when the explosion happened. It also includes eight new citations for the mine, three of which list reasons that caused the deaths of all five men. One citation said open flame torches were used in an area of the mine where methane levels were high.
