Investigator Says Comair Crash Exposed System Failures

The lead investigator of a deadly Kentucky plane crash agrees the pilots were to blame.

National Transportation Safety Board member Deborah Hersman also said the accident exposed failures across the nation's aviation safety system.

The National Transportation Safety Board voted in Washington in July that pilot error was the primary cause, and the Federal Aviation Administration had a lesser role.

The Comair jet crashed in the pre-dawn darkness Aug. 27, 2006, shortly after the pilots took off from the wrong runway.
